We can all say the American people are to blame, or the partisan congress, or the Republicans, or maybe even some Democrats. (Keep in mind that I am a Republican, or at least what is supposed to be a conservative) However, all it takes are a few bad apples at the top, and those who run things in the background to really make a difference. The PNAC boys, the Trilateral Commission and the Carlye Group (among others) really knew what they were doing.
